How is my tax liability calculated?
Box 5 reports wages subject to Medicare taxes. Wages reported in box 5 is what's reported on line 1 of the City return.
If you have wages in Box 18 subject to local or city income taxes, any amount of withholding will be reported in Box 19. The City of Forest Park will allow a credit of 25% of 1.5%.
In calculating credit for tax paid to other municipalities, the credit may not exceed 1.5% of those wages actually taxed. Tax withheld at a rate higher or lower than 1.5% must be reduced to the percent allowable.
Example 1:
- If you work in Cincinnati and 2.1% Cincinnati tax is withheld, you will receive credit of .375% on Cincinnati income taxed (.25% of our 1.5% tax rate).
Example 2:
- If you work in Mason and 1.12% Mason tax is withheld, you will receive credit of .28% (.25% of Mason's 1.12% tax rate)
Credit must be adjusted for any refunds from another city/county.
Please note: No credit is given for tax paid to school district taxes.
What is the due date for filing my tax return?
The deadline to file your tax return is April 15th. If that date falls on a Saturday or Sunday, then the due date will change to the next business day following the Federal holiday of Emancipation Day celebrated on April 16th.
What is the individual income tax rate in Forest Park?
The tax rate is 1.5%. Beginning in 2013, the allowable credit has been reduced over 2 years.
Effective 1/1/2013, Residents who work outside Forest Park and have another city's tax withheld (example: Cincinnati), your allowable credit towards your Forest Park return has been reduced. The new credit allows for a maximum credit of 1.125% of taxes paid to another municipality based on a tax rate of 1.5% or higher. If the tax rate of the other municipality is less than 1.5%, the taxpayer would only receive a credit up to 75% of that city's tax rate.
- Example:
- In 2013, a Forest Park resident works in Cincinnati, ears $50,000/year
- Person has Cincinnati tax of 2.1% withheld from paycheck totaling $1,050
- Forest Park will now only allow a credit up to 1.125% of $50,000, or $562.50
- Total Forest Park liability: $50,000 x 1.5% = $750 - $562.50 = $187.50 balance due

- Total Forest Park liability: $50,000 x 1.5% = $750 - $187.50 = $562.50 balance due
Who must file an individual income tax return?
Each resident over the age of 18 must file a Forest Park income tax return. The term "resident" includes the following:
- College students temporarily away from home
- Military personnel whose permanent residence is in Forest Park
- Renters
- Individuals living with property owners
Filing is required even if no Forest Park tax is due and if no Federal tax return is required.
I lived in Forest Park for only part of the year. Do I still need to file?
Yes, you must file a forest Park income tax return for the portion of the year that you were a resident.
Are there any exceptions to the filing requirement?
No. Some forms of income are not taxable, but you must still file a tax return. Non-taxable income sources include:

- Dividends
- Annuities
- Workers compensation
- Social security
- Pensions unemployment insurance
- Supplemental unemployment

Will the Tax Department assist me infiling my return?
Yes! The forest Park Income Tax office is open year round for taxpayer assistance from Monday through Friday 8:30a.m. - 4:00p.m. During the tax season (February 1st - April 15th) the Tax Office is open Monday through Friday from 8:30a.m. - 4:00p.m. with the following additional extended hours:
- Mondays and Thursdays (April 2nd, 6th, 9th, & 13th 2020) from 8:30a.m. - 6:00p.m.
- Wednesday (April 15th 2020) from 8:30a.m. - 6:00p.m.
- Saturdays (April 4th & 11th 2020) from 9:00a.m. - 2:00p.m.

When are tax payments due?
All residents must file a return, or request an extension by April 15th. Quarterly payments of 25% of the estimated tax are due on April 15th, June 15th, September 15th, and January 15th.
Can a return be amended after it's been filed?
Yes, the statute of limitations for amending a return is 3 years. An amended return must be filed to report additional income and to pay any additional tax due, or to claim a refund of overpaid taxes.
