Recreational Reimbursement Program
 
*Attention!  Submission for Recreational Reimbursements will begin January 1, 2012.
 
The Recreation Department encourages an active & healthy lifestyle and is offering a new program to assist Forest Park residents!!  Forest Park residents can now receive reimbursement for recreational programs and memberships to the listed recreation and fitness facilities below.  Let's get moving Forest Park!
 

How Does the Program Work?

Residents who purchase new or renewed memberships and/or participate in programs (including summer camp), at the facilities listed below, can receive reimbursement to participate in a combination of programs and/or memberships up to a maximum amount of $100 per household for any combination from November 2, 2010 - November 1, 2011.  For example, a resident can purchase a one-year family membership to the YMCA and receive up to $100 reimbursement.

  • Greenhills Community Pool - (589-3585)
  • Tri-County Soccerplex - 530 Northland Boulevard (825-3533)
  • Waycross Athletic Club - 1990 Waycross Road (742-1700)

 

REIMBURSEMENT PROCESS

The number of reimbursements is limited & will be on a first-come-first-served basis.  

Residents will be able to submit reimbursement requests beginning January 1st, to the Forest Park Finance Department.  All reimbursement requests must be received in the Forest Park Finance Department by November 1st at 4:00 PM.  All reimbursement checks will be processed on or around November 15th, provided submissions are received by November 1st. Reimbursements are paid ONLY to the resident providing payment. **Electronic submissions will not be accepted** PLEASE BE AWARE: Anyone delinquent on municipal taxes, fines, etc will not qualify to participate in this reimbursement program. 

 

What Items are Needed to Receive Reimbursement?

  • Proof of Payment - Receipt(s) AND copy of check (front & back), copy of credit card statement, copy of money order stub, or copy of bank statement for electronic withdrawals.
  • Proof of Residency - A current bill with your name & address from Duke Energy, Rumpke, Cincinnati Water Works, Cincinnati Bell, Stormwater Utility, or Time Warner Cable. 
  • Completed Reimbursement form - located in the Forest Park Finance Department or click here.
